Palliative Care Isn't the End of Heart Failure Care. It's an Essential Part of It.

One of the biggest misconceptions in cardiology is that palliative care is only for patients at the very end of life. I would argue the opposite.

I would argue the opposite.

For patients living with heart failure, palliative care should be viewed as another pillar of comprehensive, patient-centered care. It exists alongside guideline-directed medical therapy, advanced heart failure interventions, and multidisciplinary management—not in place of them.

Heart failure is a lifelong journey marked by recurrent symptoms, frequent hospitalizations, difficult decisions, and tremendous emotional and physical burdens for both patients and their families. While we continue to optimize medications, devices, and advanced therapies, we must also address what matters most to our patients: symptom relief, quality of life, independence, dignity, and care that aligns with their goals and values.

Palliative care provides:

• Symptom management

• Goals of care and shared decision-making conversations

• Advance care planning

• Emotional and psychosocial support for patients and caregivers

• Thoughtful end-of-life planning when appropriate

At Sharp Chula Vista Medical Center, we have intentionally integrated palliative care into our multidisciplinary heart failure program. Rather than viewing it as a last resort, we see it as an important component of comprehensive heart failure care that works alongside evidence-based therapies to support patients through every stage of their disease.

This integrated approach has helped us deliver more patient-centered care, improve the care experience for patients and families, and ensure that treatment decisions remain aligned with each patient's goals and values.

As heart failure specialists, success should not be measured only by how long our patients live, but also by how well they live.

The best heart failure programs don't simply treat the failing heart. They care for the whole person.
